(Conveniently located in our Mapleton Center) The most comprehensive collection of rehabilitation services in the Boulder Valley is conveniently located at Boulder Community Hospital. We evaluate and treat individuals of all ages. Patients benefit from a full range of services available to both inpatients and outpatients. These include physical, speech and occupational therapies, counseling, case management and nursing. Experts from many disciplines work together to meet patients' physical, emotional, social, cognitive and vocational needs. This coordinated team approach helps to speed recovery and allows each patient to gain as much independence as possible. Rehabilitation Services: If you require continued care, rehabilitation programs and/or home health care options are considered. Some patients qualify for transfer to the hospital's rehabilitation program. This comprehensive program features three hours per day of the appropriate therapies. Other patients are transferred to a skilled nursing facility for further rehabilitation. Once patients can function independently and safely, they return home and continue rehabilitation from there. They may participate in rehabilitation programs at the Mapleton Center, or if they are homebound they may use the services of Boulder Community HomeCare registered nurses, therapists and home health aides.
