Family practice physicians handle a wide variety of medical problems and care for patients of all ages, often as a family group. She stayed on at UCHSC for her residency training in family medicine. Dr. Duiker is a board-certified family practitioner with broad academic experience. She serves on the clinical faculty at the UCHSC, and previously held a similar post at the University of Texas Health Science Center at Houston. While in Texas, she was a multiyear recipient of the Dean’s Teaching Excellence Award. She has also served as the chair for the Department of Family Practice at Children’s Hospital in Denver. Dr. Duiker’s teaching experience has been complemented by a long history of publishing clinical research and giving presentations at state and national medical conferences. Dr. Duiker has practiced family medicine locally since 1989. She provides complete medical care for families and has additional expertise in low-risk obstetrics. David Hibbard, M.D. Dr. Hibbard received his medical degree in 1966 from Cleveland's Case Western Reserve University, where he received the university's prestigious award "For Excellence In The Care Of Patients." Dr. Hibbard has been practicing family medicine locally since 1974. Prior to that, he worked as a physician in Kenya and as a Peace Corps physician in India. In 1980, he co-founded the Family Medical Center with his wife, Chris Hibbard, Ph.D. Dr. Hibbard prides himself on being able to treat all members of a family. His background includes specialty training in internal medicine, obstetrics-gynecology and geriatrics, and he's board-certified in pediatrics. He has staff privileges at both Boulder Community Hospital and Avista Hospital. Dr. Hibbard and his wife are currently writing a book about end-of-life decision making. They have three children. Mira Lee, M.D. Dr. Lee received her bachelor’s degree in biology from the University of Toledo and her medical degree from Ohio State University. She completed her three-year family practice residency at Ravenswood Medical Center, Chicago, and the University of Illinois at Chicago. During her third year, Dr. Lee served as Chief Resident. She also finished a faculty development fellowship through Cook County Hospital and Rush University. After practicing family medicine locally for six years, Dr. Lee joined Family Medical Associates of Lafayette in November of 2004. Dr. Lee’s philosophy of patient care is focused on helping patients realize optimal health by collaborating with them. Dr. Lee is a board-certified family practitioner with additional expertise in low-risk obstetrics. She belongs to the Boulder County Medical Society, American Academy of Family Practice, and Colorado Academy of Family Physicians. Conversational in both Spanish and Korean, Dr. Lee enjoys working out, eating different cuisines, traveling and spending time with family and friends. She is married and has a son. Adelina Longoria, M.D. Dr. Longoria received her medical degree from the University of New Mexico in 1997. While attending medical school, she received both an Excellence in Research award and a National Fellowship award. Her undergraduate degree, a B.S. in Microbiology, is from New Mexico State University. Fluent in Spanish and English, Dr. Longoria is a member of the American Medical Association and the American Academy of Family Physicians. Dr. Longoria has staff privileges at both Boulder Community Hospital and Avista Hospital. Shawn Otteman, D.O. As an osteopathic physician, Dr. Otteman stresses a holistic approach to family medicine. He enjoys treating the whole person, often incorporating osteopathic manipulative techniques into patient care. Dr. Otteman completed his undergraduate degree at the University of Colorado at Boulder in 1984. He received his Doctor of Osteopathic Medicine (D.O.) degree from the Chicago College of Osteopathic Medicine in 1991. Dr. Otteman completed his family practice residency training at the University of Cincinnati. He served as Chief Resident during his second year. Dr. Otteman joined Family Medical Associates of Lafayette in July of 2003. Previously, he practiced at People’s Clinic, which provides high-quality health care for low-income Boulder residents. Sharon Pittenger, M.D. Dr. Pittenger received her medical degree from Michigan State University in 1985. Her undergraduate degree is a B.A. from Ohio's Denison University. She also completed three years of pre-med work at Eastern Michigan University. She has been practicing family medicine locally since 1990. Dr. Pittenger is certified by the American Board of Family Practice and is a member of the American Academy of Family Physicians and the Colorado Academy of Family Physicians. She has staff privileges at both Boulder Community Hospital and Avista Hospital. Dr. Pittenger and her husband, Mark, have a daughter, a son, a cat and a dog. She enjoys hiking, dancing, cooking and attending her children's sporting events. Kristen Royer, M.D. Dr. Royer is a board-certified family practice physician. After receiving an undergraduate degree in biology and Spanish from Stanford University, Dr. Royer earned her medical degree from the University of California Los Angeles (UCLA). She completed her residency at Ventura County Medical Center in 1988 and soon afterward began practicing family medicine in the Boulder area. She joined Family Medical Associates in November of 2004. Dr. Royer provides complete medical care for all ages and has additional expertise in low-risk obstetrics. She stresses disease prevention and strives to form in-depth relationships with patients by caring for every member of the family. In 2000, Dr. Royer was named a Fellow of the American Academy of Family Physicians, recognizing her service to family medicine and her personal commitment to medical education. Carolyn Sanders, M.D. After earning a bachelor’s degree in biology with honors and distinction from Stanford University, Dr. Sanders received her medical degree from Harvard Medical School. She completed her family practice residency at Cleveland Metropolitan General Hospital in 1986. Dr. Sanders joined Family Medical Associates of Lafayette in November of 2004. She has practiced family medicine locally since 1992. She provides complete medical care for families, especially multi-generational families, focusing on health maintenance, wellness and disease prevention. Dr. Sanders also offers special expertise in low-risk obstetrics. As a family practitioner, Dr. Sanders thinks of herself as an educator, building relationships with patients and helping them make choices that put their health first. Melissa Van Tassel, M.D. A board-certified family practitioner, Dr. Van Tassel is a Dayton, Ohio Wright State University School of Medicine graduate. She completed her residency at St. Anthony Central Hospital in Denver. Her particular interest is women's health and adolescent medicine. She treats patients for a complete range of illness and injuries and also has specialized training in colposcopies and other gynecological procedures. She is certified in cardiac, obstetric and neonatal advanced life support. Dr. Van Tassel's personal interests include cooking, traveling and hiking. Terrell Webb, M.D. Dr. Webb received his medical degree from The Medical College of Ohio at Toledo. He completed his internship and residency at the Aultman Family Practice Residency Program and Aultman Hospital in Canton, Ohio. Dr. Webb has an undergraduate degree in Psychology from Wright State University in Dayton, where he graduated Summa cum Laude with Departmental Honors. Dr. Webb is a Board Certified family practice physician who specializes in adult medicine. He was born and raised in Dayton, Ohio. He has many outside interests including flying RC aircraft, dive medicine, and spending time with his family. Stephen Zakany, M.D. Dr. Zakany received his medical degree from the University of South Florida in 1982. His undergraduate degree in Chemistry is from Florida Atlantic University. Dr. Zakany is board certified in Family Medicine, and has been with Community Medical Associates since 1999. Prior to joining CMA, Dr. Zakany spent two years in Germany. He has also practiced in Neptune Beach, Florida and Durango, Colorado. Specific areas of interest include sports medicine and pediatrics. A native of Colorado and former ski patroller, Dr. Atkins enjoys mountain running, skiing and bicycling Scott Daarud, M.D. Dr. Daarud received his medical degree from Loma Linda University in 1979. His undergraduate degree, a B.S. in Natural Science, is from Union College in Lincoln, Nebraska. Dr. Daarud has been practicing family medicine in Boulder County since receiving his certification from the American Board of Family Medicine in 1983. Dr. Daarud has a special interest in back disorders, asthma and preventive medicine. He is a member of the American Academy of Family Physicians, the Colorado Medical Society and the Alpha Omega Alpha Honor Medical Society. Raised in Boulder, Dr. Daarud is an avid golfer and mountain biker. He is married and has two sons. Frances Hamilton, M.D. A board certified family practitioner who has practiced in the Boulder Valley since 1995, Dr. Hamilton is a Dalhousie Medical School (Nova Scotia, Canada) graduate who completed her residency at Wilson Memorial Hospital in Johnson City, New York. While in medical school, she was elected into the Alpha Omega Alpha Honours Medical Society. As a family physician, she sees patients of all ages, and is open to blending Western and complementary medical approaches. She has traveled to India several times and has volunteered her medical services there. A former high school principal, Dr, Hamilton's personal interest include mountain sports, music, yoga and reading. She lives with her husband and their daughter. Anna Rooney, M.D. Dr. Rooney received her medical degree from New York's Albany Medical College in 1995. Her B.S. is a double major in Biology and Studio Arts from Union College in Schenectady, New York. She was both Magna Cum Laude and Phi Beta Kappa during her undergraduate studies. She is certified by the American Board of Family Practice and has a special interest in preventative medicine. Prior to joining the Gunbarrel Medical Center, she practiced family medicine in Niwot, and served as a consultant for a medical information web site. Dr. Rooney is a member of the American Association of Family Practitioners and the Colorado Association of Family Practitioners. Dr. Rooney is married and has three sons. In her spare time, she enjoys painting and downhill skiing. David Simons, M.D. Dr. Simons received his medical degree from the University of Texas in 1971. His undergraduate degree is from Sam Houston State College in Huntsville, Texas. A co-founder of the Gunbarrel Medical Center, Dr. Simons has been practicing family medicine locally since 1980. Prior to that, he practiced in Texas and California † serving two years at the U.S. Navy's Regional Medical Center in Long Beach. Dr. Simons is certified by the American Board of Family Medicine. A native of Texas, Dr. Simons enjoys skiing and hiking. He is married and has a son and two daughters. V. James Thompson, M.D. Dr. Thompson received his medical degree from the University of Texas in 1966. A co-founder of the Gunbarrel Medical Center, Dr. Thompson has been practicing family medicine locally since 1980. Prior to that, he practiced in Minnesota and North Dakota - serving two years with the U.S. Public Health Service's Division of Indian Health. Dr. Thompson is certified by the American Board of Family Medicine and is a member of the American Academy of Family Physicians. A native of western North Dakota, Dr. Thompson enjoys golf and mountain sports. He and his wife, Linda, have two daughters. After serving Boulder Valley residents in his solo practice for two decades, Dr. Beasley joined Table Mesa Family Medicine in 1990. He is certified by the American Board of Family Practice and is a member of the American Academy of Family Physicians. Dr. Beasley and his wife, Barbara, teach medicine and nursing in the former USSR and volunteer their services in Mexico and Haiti. Stephanie Buening, M.D. A Cincinnati native, Dr. Buening graduated from the Ohio State University College of Medicine in 1999 and completed her residency at the University of Colorado in 2002. She practiced a full range of family medicine in Loveland before joining Table Mesa Family Medicine in 2004. Board certified in family medicine, she has a special interest in women's health, preventive health and nutrition. She is a member of the American Academy of Family Practice and a clinical professor at the University of Colorado Health Sciences Center. Br. Buening believes the journey to optimal health takes a team approach, and the patient must be informed and involved. She embraces complementary and alternative medicine, acknowledging Western medicine's limitations and the importance of a patient's attitude toward healing. She enjoys outdoor activities such as skiing and sailing as well as writing and reading. Alex Bulkacz, M.D.
Dr. Bulkacz has been treating patients in the Boulder Valley for the past 14 years. He brings expertise in preventive cardiology and diabetes care to our team. Dr. Bulkacz received his undergraduate degree from the University of California, Los Angeles, and went on to receive his medical degree at the Medical College of Pennsylvania in Philadelphia. He completed his internship at the University of Colorado Family Practice Program in 1994, and his residency at Lafayette's Clinica Campesina Family Practice Program in 1996. Prior to joining Table Mesa, Dr. Bulkacz served as a family practitioner at Clinica Campesina and People's Clinic, and as a clinical instructor in the family medicine department at University Hospital in Denver. He is married with two young children. In his spare time, he enjoys playing and listening to live music, and participating in a variety of outdoor activities. William Burton, M.D. Dr. Burton received his medical degree from the University of Colorado in 1981. He has been practicing family and sports medicine at Table Mesa Family Medicine since 1986. Dr. Burton is certified by the American Board of Family Practice and is a Fellow of the American Academy of Family Physicians (the organization’s highest educational distinction award). He is also board certified in sports medicine.Dr. Burton frequently sees patients at the Boulder Center for Sports Medicine. A native of Georgia, Dr. Burton enjoys running, hiking, snorkeling, snowborading and Shotokan Karate. He and his wife, Jenny, have two sons and a daughter. Zara Gershberg, M.D. Dr. Gershberg is a board-certified, family practice physician and a strong proponent of preventative medicine. She is a member of the American and Colorado Academies of Framily Practice, as well as the American Medical Association. In her spare time, Dr. Gershberg enjoys hiking, skiing, camping, yoga and painting. Sue Griffith, PAC A physician assistant and child health associate since 1986, Ms. Griffith provides primary health care to infants, children and adults. She has practiced at Table Mesa Family Medicine since 1998. Previously, she was a primary family practice provider at the Peoples Clinic in Boulder. Ms. Griffith is accredited by the National Commission on Certification of Physician Assistants and is a member of the Colorado Academy of Physician Assistants. She received her masters degree from the University of Colorado’s Child Health Associate Program in 1986. Ms. Griffith enjoys cross country ski racing and other outdoor activities with her family. She and her husband, Ray, have two children. She is fluent in conversational Spanish. Gerald Hickman, M.D. Dr. Hickman received his medical degree from the University of Colorado in 1965. His association with Table Mesa Family Medicine dates back to 1970. Prior to that, he served with the U.S. Navy. Dr. Hickman is a member of the American Academy of Family Physicians, the Colorado Academy of Family Physicians, the Colorado Medical Society and the Boulder County Medical Society. In addition to his family medicine duties, Dr. Hickman regularly performs FAA exams for pilots. A native of Colorado, Dr. Hickman enjoys cycling, goldsmithing, oenology and photography. He and his wife, Doree, have a son. Dr. Hickman encourages his patients to take responsibility for their health. Carol Holdcroft, N.P. Carol has been a Family Nurse Practitioner since 1982, providing primary health care for infants, children and adults. She joined Table Mesa Family Medicine in June of 2007. Previously she worked as a N.P. at the Denver VA Medical Center, University Medicine, People's Clinic and Mountain Family Health Center. Ms. Holdcroft is certified as a Family Nurse Practitioner by the American Nurses Credentialing Center. She received her Master's Degree in the Nurse Practitioner program at University of Colorado Health Sciences Center in 1982. Carol enjoys caring for patients of all ages, but has a special interest in caring for older adults. She likes downhill and cross country skiing, hiking, biking and riding motorcycles.
